  Lipoma  is a painless, fatty mass in the deep layers of the skin. It is benign and causes no other health problems. The skin above a lipoma is normal.


Lipomas are very common and vary in size. They arise most often on the upper body and arms, but they can occur anywhere on the body where fat tissue is found. 

The cause of lipomas is unknown, but it appears that some people inherit the tendency to develop lipomas. Some people develop lipomas in areas of injury
or trauma.

No treatment is recommended unless the lipoma becomes a cosmetic concern, becomes tender, or bleeds from friction against clothing.



Treatment options include intralesional steroid injections and surgical removal.

The conventional means of lipoma removal is open excision. While effective, it does leave scars. Laser liposuction can be used to treat lipomas but you can't think of it as traditional liposuction as you can not really suction out lipomas like regular fat. The laser probe can be inserted into a lipoma and turned out to help melt or liquify the lipoma and then allow the body to absorb the broken down lipoma.

This  is a lipoma removal technique that is in a state of evolution and is not yet proven to be as consistently effective as excision even  though it is more scarless.

We have performed lipoma excision with the LightScalpel CO2 laser. It's a short procedure, where a small straight incision is made in the skin, the lipoma is then extruded and quickly undermined and it is almost scarless. 

Treatment
NONEXCISIONAL TECHNIQUES
Non excisional treatment of lipomas, which is now common, includes steroid injections or liposuction.


Steroid injections result in local fat atrophy, thus shrinking  the lipoma. A one-to-one mixture of 1 percent lidocaine (Xylocaine) and triamcinolone acetonide (Kenalog), in a dosage of 10 mg per mL, is injected into the center of the lesion; this procedure may be repeated several times at monthly intervals.

The volume of steroid depends on the size of the lipoma, with an average of 1 to 3 mL of total volume administered. The number of injections depends on the response, which is expected to occur within three to four weeks. Complications, which are rare, are the result of the medication or the procedure, and can be prevented by injecting the smallest total amount possible and by positioning the needle so that it is in the center of the lipoma.

Liposuction

Because lipomas are made of fatty tissue, liposuction can be utilized as way to get rid of most of their mass

Older Dermatologists like myself rely on history and physical to a greater degree than the younger dermatologists.  I think that the pendulum has swung too far towards all these cosmetic procedures where the younger dermatologists are just doing cosmetic procedures and not listening to their patients.  This is an important point because if the Dermatologist does not spend enough time with the patient the cosmetic procedures will not work properly.
